Acute kidney injury (AKI) is a common syndrome in hospitalized patients and has consistently been associated with increased attributable short-term and long-term morbidity and mortality, and increased consumption of health resources. Accordingly, the early detection and diagnosis of AKI in hospitalized patients is a clinical priority. Likewise, a method to determine the etiology of AKI and grade its severity, in terms of injury and loss of function, are vital to enable appropriate triage, to guide therapeutic interventions and to provide prognostic information (for example, the trajectory of decline in renal function, probability of requirement for renal replacement therapy and recovery of kidney function). Perazella and colleagues5 have investigated the value of urine microscopy in the differential diagnosis of AKI.
