Inferior Subcoracoid Acromiociavicular Dislocation

Inferior subcoracoid acromioclavicular joint dislocation is extremely rare. Only four adult cases without associated fractures have been reported. The dislocation occurs owing to high-energy trauma with forceful abduction and external rotation. Open reduction and joint restoration have been recommended.Case Study: Subcoracoid dislocation occurred in an 18-year-old woman. The clavicle was firmly unhooked under the coracoid process. Previously unreported in treatment of inferior dislocation, osteotomy of the coracoid process was necessary to reduce the displacement. Two Kirschner wires were inserted to secure the reduction and maintained for 6 weeks. Except for the torn coracoclavicular ligaments, the disrupted soft tissues were reconstructed. Normal, pain-free function was regained by 12 weeks and was unchanged at 11 years follow-up.
