What does a diagnosis of prediabetes mean for weight management? Findings from the 3D study

Background: Prediabetes is a state of physiologically increased risk of Type 2 Diabetes (T2D), cardiovascular and renal disease. People with obesity are seven times more likely to develop T2D, compared to lower weight individuals. Improving diet quality and reducing body weight can improve glycaemic control and delay T2D. However, it is unclear how patients perceive the role of weight in prediabetes management. Aim: To explore the care experiences for weight management of people diagnosed with prediabetes. Methods: Qualitative, semi-structured interviews were conducted, with a sub-sample of participants from The 3D Study (Diet after Diagnosis of Diabetes), a national case-series study of diet and diabetes. Participants were adults newly diagnosed with T2D who had previously been diagnosed with prediabetes. A deductive approach using pre-specified codes related to weight and diet was used to reveal themes. Themes were discussed by all investigators until consensus was reached. Results: Interviews were conducted with 20 people (8 males, 12 females), with interview length ranging from 30 to 60 minutes. Four themes emerged related to weight management: i) heightened awareness of weight and diabetes link; ii) variable success with weight loss attempts; iii) weight loss is perceived as positive and motivating; and iv) receiving weight loss care. Conclusions: While patients were aware of the association between weight and T2D risk they had received limited weight management support during prediabetes. Patients could benefit from a consistent approach to prediabetes diagnosis, weight management and dietary support.
