Osteoarthritis: is this a disease for biologies or the physiatrist?

Development of treatment paradigms for patients with osteoarthritis is dependent on an understanding of the pathogenesis of the disease process. The destruction of tissue that is accompanied by remodeling and hypertrophy of surrounding bone as well as varying degrees of synovial inflammation appears to be dependent on multiple risk factors, including hereditary contributions, biomechanical factors, intraarticular crystal deposition, obesity and the effects of aging . It is possible that predominant pathophysiologic effects may define different phenotypes of osteoarthritis and therapies that target these specific phenotypes may be needed. In order to slow or halt the development of structural changes in osteoarthritis joint tissue, a clear understanding of the phenotype of the patient may be necessary in order to target disease progression
