Djamel Mokart and Marc Leonepoint out potential diffi culties forinterpreting procalcitonin con centrationsin immunocompromisedpatients. In our trial, these patientshad signifi cantly lower antibioticexposure in the procalcitonin groupthan in the control group (3?6 days),without signifi cant diff erences inmortality and relapse rates. However,we agree that more trials are neededto confi rm that a procalcitonin-guidedstrategy can reduce antibiotic usewithout adversely aff ecting outcomesin that specifi c subset of patients.
