In their Viewpoint, Cordelia Coltart and colleagues (May 12, p I847) focus on the substantial effect of leadership development programmes, such as the Chief Medical Officer's Clinical Advisors Scheme, within the National Health Service in England. They reflect that a wider culture of leadership development across the health-care system "is likely to be difficult to deliver".We believe the answer has to come from strengthening support for doctors in clinical placements. In their early years, doctors rotate through many different departments. Each offers different opportunities for work-based leadership learning, provided that there is a culture of improvement, where ideas, insights, and innovation from all staff are valued.
