Reconciling medicines for patients

Patient safety has been improved through a project where primary care pharmacists were involved in medicines reconciliation for patients transferring between care settings. The project — a finalist in this year's Pharmaceutical Care Awards — included primary care pharmacists from NHS North of Tyne who worked to link GPs, community pharmacists, and hospital prescribers and pharmacists, when patients were discharged from secondary care.
