A visiting care service

Stephen Gabell (PJ,31 October 2009, p482) makes an important point that community pharmacists should be more involved in the pharmaceutical care of patients with long-term illnesses, and I would add particularly with housebound elderly patients. An elderly man I have been visiting in a personal capacity over the past year died recendy. Although he was being visited by a nurse and care workers, it became apparent to me that he was confused about his medicines, particularly his asthma treatment, which he was neglecting.
