LifeSpan Comprehensive Services with Home Visiting

This project seeks to meet the developmental needs of pregnant and parenting211u001eadolescents, and promote successful health outcomes for their infants. The 211u001eproject combines education and support components of LifeSpan-an established 211u001eprogram for pregnent and parenting adolescents-with case management services, 211u001eincluding home visiting by trained paraprofessionals. LifeSpan is an early entry 211u001epoint into medical and social service supports for pregnant adolescents. 211u001eComprehensive services will ensure that mothers and infants receive a continuum 211u001eof care.
