In September, 2005, a 41-year-old man came to our emergency department with a sore throat. Swallowing had been difficult and painful for 2 days. He also felt short of breath, especially when lying down. He had been given antibiotics by his primary-care physician, but these had been ineffective. The patient had misused alcohol for years, and had hepatitis B. He had cirrhosis, and had bled on several occasions from oesophageal varices.
