Tissue extraction by morcellation: A clinical dilemma

In recent months attention has been focused on the safety of power morcellation after a physician in Boston with an occult uterine sarcoma underwent a hysterectomy with uterine morcellation. The poor prognosis associated with uterine sarcomas is worsened when a uterine sarcoma is morcellated [1]. The clinical dilemma is whether we treat all symptomatic uterine leiomyomas as cancer until proved otherwise. If so, methods of treatment that do not provide hi stologic confirmation of benign disease such as uterine myoma embolization or magnetic resonance-guided focused ultrasound should also be reconsidered. It is unclear whether these forms of treatment worsen the prognosis. The focus of attention, however, is on morcellation, in particular power morcellation, reviewed in this edition of the Journal of Minimally Invasive Gynecology.
