Plasmacytoma of Liver Mimicking Hepatocellular Carcinoma at Multiphasic Computed Tomography Evaluation

A 76-year-old man presented to the surgeon's office with periumbilical abdominal pain of greater than 1-year duration. He reported previously smoking 2 packs of cigarettes a day in early adulthood, quitting 40 years ago, and he denied drinking alcohol. Liver function tests were normal. Single-phase contrast-enhanced computed tomography (CT) of the abdomen and pelvis, acquired in the portal venous phase, was performed at an outside institution before the presentation to the surgeon's office. This examination showed a hypoattenuating mass at the dome of the right hepatic lobe. A previous single-phase contrast-enhanced CT of the abdomen and pelvis performed in 2009 showed no mass in the same area. The patient also had a history ofarrhythmia that was treated with cardiac pacemaker; this is a contraindication to magnetic resonance imaging (MRI) at our institution. Therefore, multiphasic CT of the abdomen was ordered to further evaluate the liver mass.
