Peripheral iris transillumination defect and recurrent anterior chamber bleeding with bag-fixated intraocular lens

A 74-year-old man was referred for intermittent visual deterioration because of recurrent spontaneous anterior chamber bleeding in the left eye. The bleeding episodes started 1 year ago, and although they had increased in intensity, they spontaneously cleared within days. A recent hyphema reportedly filled one third of the chamber and caused severe visual loss and a significant increase in intraocular pressure (IOP), finally prompting referral to our institution. The patient had cataract surgery in the left eye 10 years earlier with implantation of a foldable intraocular lens (IOL) (Ceeon Edge 911A, Pharmacia) with a 6.0 mm sharp-edged silicone optic and 12.0 mm looped haptics angulated by 6 degrees. The haptics are polyvinylidene fluoride (PVDF) and exit the optic in a 90-degree angle to then bend rather abruptly, following the capsular bag contour (capsule C design)
