CASE 7 - 2012 airway management and perioperative decision making in the patient with severe pulmonary hypertension who requires emergency noncardiac surgery

ALTHOUGH HEART FAILURE and arrhythmias are the principal causes of death in severe pulmonary hypertension (PH), noncardiac surgery constitutes a high-risk intercur-rent event. Optimal perioperative care for patients with severe PH must attend to the physiologic and logistic challenges of critical illness and emergency noncardiac operative intervention superimposed on those with severe PH. The authors present the case of a morbidly obese patient with severe PH who required emergency noncardiac surgery and discuss the strategies of airway management and perioperative intervention as they apply to this high-risk population.
