Simultaneous Left Ventricular Pseudo-Aneurysm and Ruptured Ventricular Septal Defect Following an Acute Myocardial Infarction

A 72-year-old male presented with congestive heart failure following an acute myocardial infarction three weeks prior to admission. Catheterization revealed total occlusion of the right coronary artery and a large pseudo-aneurysm of the left ventricle (Fig. 1). A trans-esophageal echocardiogram also demonstrated a ventricular septal defect (Fig. 2). At time of surgery, a periinfarct ventricular septal-defect was closed with a bovine pericardial patch, and a second patch was used to repair the pseudo-aneurysm. The patient tolerated the procedure well and has remained asymptomatic five months following surgery.
