Basal cell carcinoma rebound after cessation of vismodegib in a nevoid basal cell carcinoma syndrome patient

Effective management of patients with nevoid basal cell carcinoma syndrome (NBCCS) can pose a challenge for Mobs surgeons. Research has been directed at inhibiting the sonic hedgehog (SHH) pathway in patients with advanced basal cell carcinoma (BCC). We report a patient with NBCCS unable to tolerate vismodegib during a clinical trial who experienced a rapid resurgence of BCC beginning 3 months later. Multiple sessions of Mohs micrographic surgery (MMS) were necessary to control the patient's disease.
