Explosions, car wrecks and workplace violence: these are events that grab headlines. What is interesting, however, is that the most mundane things are often the most dangerous. In the realm of hydrocarbon processing industry (HPl) plant safety, that danger lies in dropped tools. Widely publicized and universally feared toxic gas releases, massive explosions or pipeline leaks, while incredibly destructive and certainly the result of some errors or omissions in a well-run safety program, are actually not the top causes for workplace incidents and fatalities.
