Management of OPPORTUNITY IN HOME HEALTHCARE

What Is the Premise of Disease Management?The Disease Management Association of America (DMAA), The National Committee on Quality Assurance, and other organizations have defined disease management using certain common elements. These definitions view disease management as an approach to delivering healthcare to persons with chronic illnesses that aims to improve patient outcomes while containing healthcare costs (USDHHS CMS, 2003). Measuring the effectiveness of care interventions and costs associated with these interventions is central to disease management. Disease management depends on outcomes management: the ongoing measurement of outcomes and their correlation with treatment (Epstein & Sherwood, 1996). The measurement of outcomes provides actual results of care rendered. Providers need these results to know the level of effectiveness that their interventions have achieved and what, if any, revisions need to be made to enhance care provision and services (Huffman, 2005).
