We are grateful to the World of Journal of Surgery (WJS) for providing a forum for this scientific discussion. In their letter about our article published in the September 2011 issue, Sudhi Agarwal, Amit Agarwal, and Gayan Chand raise four questions about our work [1]. We appreciate their interest in our study and are prepared to respond to the important points they bring to the discussion. We are so pleased that they agree with us about the importance of using intraoperative parathyroid hormone (PTH) to mange hyperparathyroidism patients, especially those who have a normal PTH level from the start. As they said, very few studies have addressed this issue, although it is well recognized that it presents in the majority of cases worldwide.
