Declaration of competence forms:cut-off date soon

Pharmacists are reminded that the cut-off date for the submission of declaration of competence forms for dispensing and pharmacy assistants is 31 December (PJ,4 September,p331).A completed form should be submitted for each member of staff in current employment who falls within the minimum competence requirement grandparent clause.The forms should be sent to the Registration Section,Royal Pharmaceutical Society,1 Lambeth High Street,London SE1 7JN.
