Editorial comment. Impact of prostate weight on probability of positive surgical margins in patients with low-risk prostate cancer after robotic-assisted laparoscopic radical prostatectomy.

Multiple studies have shown that there is a higher rate of positive margins in men with small-volume prostates who undergo radical prostatectomy. Some of this seems to be from a higher risk of extraprostatic extension in smaller prostates, but part of it may be technical. Radical retropubic prostatectomy sometimes is more difficult in men with small prostates because the prostatic apex may be less distinct. With robotic-assisted laparoscopic prostatectomy, it has seemed to me that improved visualization partly overcomes difficulties in anatomic definition of the prostate and that technical issues related to small prostate size should be less influential.
