The second part of this article initially summarizes the complete protocol for the lateral cephalometric X-ray imaging technique with the patient in a supine position, describes the cephalometric method in retral and advanced mandibular positions (relative cephalometry) and demonstrates the clinical application of the lateral cephalometric X-ray as a partial predictor in the therapy of obstructive sleep apnea (OSA) with intraoral advancement splints (IPS). Relative predictors are currently internal and external otorhinolaryngeal findings, body mass index (BMI), severity grade of OSA and functional findings of the temporomandibular joint and bite type. Based on documented patient case studies it will be discussed how the single predictor lateral cephalometric X-ray should be assigned in this complex of OSA favorable methods in order to be able to achieve a higher prognostic certainty. The case studies confirm the practical importance of the results obtained from relative cephalometry and in this context the lateral cephalometric X-ray can only be a partial factor but is also, however, currently the only imaging predictor for the prognosis of the IPS effects in the therapy of OSA. Decisive is, therefore, the final assessment of all OSA predictors as a complex unit.
