After an accident, it’s usually clearthat someone made a mistake ...often more than one “someone.” Theofficial investigation, reconstruction,and analysis by the NationalTransportation Safety Board (NTSB)is less concerned with apportioningblame than identifying those points atwhich different decisions might haveinterrupted the resulting sequence ofevents. Even so, experts can and dodraw different conclusions about therelative importance of actions takenby the drama’s various actors.
