The millennium development goals for reducing maternal and child mortality will not be achieved in most developing countries Programmes emphasising skilled attendance and institutional delivery are missing the poorest populations, where most mothers deliver at home Evidence is growing that community based interventions are effective in reducing maternal and neonatal deaths in countries with high mortality Randomised, controlled trials of the community effectiveness of interventions are urgently required.
