Objective To determine the effects of glutamine on the nuclear factor kappa B (NF-kB) activity and IL-6 ex-pression in peripheral blood mononuclear cel s(PBMC) of patients with chronic obstructive pulmonary disease(COPD). Methods PBMCs were col ected from COPD patients with acute exacerbation (AECOPD group, n=30) , stable COPD patients (SCOPD group, n=30) and 15 health controls. Two COPD groups were further divided into 4 subgroups:blank control and glutamine (Gln) groups with 15 patients in each. The levels of NF-κBp65 and IL-6 gene expression in PBMCs were detected by real-time PCR. Results The expression of NF-κBp65 and IL-6 in blank control subgroups of AECOPD and SCOPD patients was higher than that in control group (P<0.01). The expression of NF-κBp65 and IL-6 in AECOPD Gln subgroup was stil higher than that in control group (P<0.05), while there was no significant difference between control group and SCOPD Gln subgroup (P>0.05);Compared with AECOPD and SCOPD blank subgroups, the expression of NF-κBp65 and IL-6 were decreased in AECOPD and SCOPD Gln subgroups (P<0.05). Conclusion Glutamine can inhibit the activation of NF-κB and down-regulate IL-6 level in PBMCs of patients with chronic obstructive pulmonary disease.% 目的通过观察谷氨酰胺(Gln)对慢性阻塞性肺疾病(COPD)患者外周血单个核细胞(PBMC)中NF-κB活性和IL-6水平的影响,探讨Gln在COPD患者治疗中的抗炎作用.方法收集30例COPD患者在COPD急性加重期(AECOPD)和COPD稳定期(SCOPD)的外周血PBMC各30例,各分为Gln组细胞培养时加入浓度为8 mmol/L的Gln和空白对照组,并以15例健康体检者作为对照组.采用RT-PCR法测定5组外周血PBMC中NF-κBp65、IL-6基因的表达.结果健康对照组NF-κBp65、IL-6表达水平低于AECOPD空白对照组(均P<0.01),与SCOPD空白对照组比较(均P<0.05).AECOPD Gln组与健康对照组NF-κBp65、IL-6比较有统计学意义(均P<0.05),而SCOPD Gln组NF-κBp65、IL-6与健康对照组比较无明显差异(均P>0.05).AECOPD Gln组与空白对照组比较NF-κBp65、IL-6的表达明显降低(均P<0.05).SCOPD空白对照组NF-κBp65、IL-6与Gln组比较表达有统计学意义(均P<0.05).结论 Gln可抑制COPD患者炎性转录因子NF-κB活性,并下调IL-6的表达水平.
