目的 研究慢加急性肝衰竭(ACLF)患者在临终前不同时段血清总胆固醇(Tch)水平的动态变化,旨在探讨血清总胆固醇(Tch)水平对慢加急性肝衰竭(ACLF)病情的预测价值.方法 采用全自动生化仪对本院187例慢加急性肝衰竭患者血清Tch水平进行检测,同时比较存活组与死亡组临终前不同时段血清Tch水平的差异.结果 慢加急性肝衰竭存活组及死亡组在临终前21～40d、11～20d、6～10d和1～5d四个时段的Tch水平(mmol/L)分别为(3.41±0.97)、(3.14±0.53)、(2.36±0.45)、(2.10±0.71)和(1.91±0.65).死亡组临终前21～40d、11～20d、6～10d、1～5d血清Tch水平逐渐降低;死亡组临终前11～20d时段与21～40d时段血清Tch水平相比较,差异有统计学意义(P<0.05).结论 血清Tch水平与ACLF病情密切相关,随着肝衰竭病程向终末期发展,Tch水平逐渐下降,血清Tch水平可以作为ACLF病情评估和预后判断的标志之一.%Objective To research the dynamics of levels of serum total cholesterol (Tch) in different period of times before the terminal phases of acute - on - chronic liver failure ( ACLF), and investigate its prognostic value in ACLF.Methods We tested the levels of serum Tch in 187 cases with ACLF by using automatic biochemical analyzer, simultaneously, compared them between survived group and death group which in different period of times before the terminal phases.Results The levels of serum Tch (mmol/L) in survived group and at 21 - 40 days, 11 - 20 days, 6 - 10 days and 1 - 5 days in death group were 3.41 ± 0.97,3.14 ±0.53, 2.36 ±0.45, 2.10 ±0.71 and 1.91 ±0.65, respectively.Compared with the survived group, the level of serum Tch was decreased gradually at four different periods of times before death.There are significant differences between the period of 21 -40 days and 11 -20 days before the teminal phases of ACLF( P ＜ 0.05 ).Conclusion Serum Tch is gradually declined during ACLF,which is closely related to tendency of illness state and prognosis.
