目的 分析慢性呼吸衰竭误诊为缺血性脑血管病的原因,以减少临床误诊.方法 回顾分析2013年1月至2015年12月收治的12例误诊病例的临床资料.结果 12例患者均经血液检验、影像学及临床表现确诊为慢性呼吸衰竭,经治疗好转11例,死亡1例.结论 误诊为缺血性脑血管病的慢性呼吸衰竭以老年患者居多,临床表现不典型,易误诊和漏诊,当影像学检查无法解释症状时,不应忽视血气分析和肺功能检查,以尽早明确诊断、及时治疗、改善预后.%Objective To analyze the reason of chronic respiratory failure which misdiagnosed with ischemic cerebrovascular disease,in order to reduce clinical erroneous diagnosis. Methods A retrospective clinical data of 12 patients misdiagnosed from 2013 to 2015 were reviewed. Results 12 patients diagnosed with chronic respiratory failure were all measured by blood test and imaging study and manifestations,11 patients were rescued successfully. Eventually one died. Conclusions The majority of cases which misdiagnosed with ischemic cerebrovascular disease are involved with old patients,whose manifestations lack of specifi city,and is easy to be misdiagnosed. When imaging study cannot explain the symptoms,we cannot ignore the exercise of blood gas analysis and lung function to improve the prognosis through early diagnosis and therapy.
