左半结肠癌合并肠梗阻患者行肿瘤切除Ⅰ期吻合术39例病例分析

摘要

Objective To investigate the clinical effect of one-stage resection and anastomosis for the intestinal obstruction due to left-sided colon cancer.Methods The clinical data of 45 patients with intestinal obstruction due to left-sided colon cancer were review.Results 39 patients were treated with one-stage resection and anastomosis,5 patients were treated by left-sided colon resection and colostomy Hartman,with closing fi stula as second phase,and 1 patients underwent palliative colostomy. All operations were successful. Incision infection occurred in 1 case.Conclusions With the improvement of perioperative management and strictselection of indications,left-sided colon cancer with bowl obstruction in patients with one-stage resection and anastomosis is safe.%目的 探讨左半结肠癌合并肠梗阻患者行Ⅰ期切除肠吻合术治疗的临床疗效.方法 回顾性分析2012年1月至2016年1月收治的45例左半结肠癌合并肠梗阻患者的临床治疗资料.结果 39例患者行Ⅰ期切除吻合术,5例患者行左半结肠Ⅰ期切除,Hartman结肠造瘘,Ⅱ期闭瘘术,1例患者行姑息性结肠造口术,手术均成功,术后仅分期手术患者中出现1例切口感染.结论 在完善围手术期处理,严格掌握适应证的情况下,左半结肠癌合并肠梗阻患者可行Ⅰ期切除吻合术.
