依托咪酯在老年患者麻醉诱导中的应用

Objectiveto observe the effect of etomidate during induction of anesthesia in elderly patients with clinical application value.Methods in our hospital from april to 2013 2013, 140 elderly patients with abdominal operation in september in our hospital, were randomly divided into etomidate group (70 cases) and propofol group (70 cases). Different effects to the patients in the 2 groups to make comparison, and evaluated the quality of life. two groups of preoperative use of etomidate or propofol for anesthesia induction, numerical analysis of BP and Hr changes during.Results the two groups of patients during anesthesia comparison, no signiifcant difference. (P >0.05) after induction with etomidate group and the propofol group BP and HR were decreased, the propofol group, BP and HR decreased signiifcantly, the difference was statistically signiifcant (P<0.05); etomidate group onset time of anesthesia with propofol group (P <0.05); slow after intubation in two groups of patients with blood pressure and heart rate rise, but there were no signiifcant difference between before and after induction.Conclusion cardiac function in elderly patients with incomplete case, etomidate can maintain a relatively stable circulatory function, suitable for induction of general anesthesia in elderly patients, not only can play the function of cardiovascular system stability, and reduce the risk of anesthesia. etomidate is more suitable for elderly patients with cardiovascular anesthesia induction.%目的：观察依托咪酯在老年患者麻醉诱导中的临床应用价值。方法回顾我院从2013年4月至2013年9月间在我院进行腹部手术的老年患者140例，随机分为依托咪酯组(70例)和异丙酚组(70例)。于2组患者术中的不同效果做出对比，并进行生存质量的评定。两组术前分别使用依托咪酯和异丙酚进行全麻诱导，分析期间BP和Hr等变化数值。结果两组患者在麻醉诱导比较中，差异无统计学意义。(P>0.05)诱导后依托咪酯组和异丙酚组的BP和Hr均有下降，其中，异丙酚组的BP和Hr下降更明显，差异有统计学意义(P<0.05)；依托咪酯组组麻醉起效时间较异丙酚组慢(P<0.05)；插管后两组患者血压和心率均上升，但诱导前后均无明显差异。结论在老年患者心功能不全情况下，依托咪酯可以保持其循环功能相对稳定，适合老年患者全麻诱导，不仅能起到稳定心血管系统功能，还有降低麻醉危险性的作用。依托咪酯更适用于老年性心血管患者的麻醉诱导。
