目的:探讨天津市不同年龄男性公安民警代谢综合征(MS)发病危险因素的异同.方法:1 250例健康体检的男性公安民警根据临床资料分为代谢综合征(MS)组和非MS(nMS)组,分析2组间肝功能、肾功能、血脂、心率、白细胞、血小板、纤维蛋白原和血沉的变化.以40岁作为年龄界线,将民警分为＜40岁组和≥40岁组,分析2组MS发病的危险因素.结果:MS组丙氨酸转氨酶、碱性磷酸酶、γ-谷氨酰转移酶、总蛋白、白蛋白、球蛋白、尿酸、总胆固醇、低密度脂蛋白胆固醇、心率、白细胞和纤维蛋白原水平均明显高于nMS组,MS组高密度脂蛋白胆固醇水平明显低于nMS组,差异均有统计学意义(P＜0.01).＜40岁民警MS发病的危险因素包括血清丙氨酸转氨酶、血尿酸、血清纤维蛋白原的升高和心率的增加,其中纤维蛋白原OR=2.056,95％CI l.125～3.759.≥40岁民警MS发病的危险因素包括血清总蛋白、血尿酸、γ-谷氨酰转移酶、白细胞的升高和心率的增加.结论:高纤维蛋白原血症是＜40岁男性民警代谢综合征的高致病危险因素.%Objective: To study the risk factors of metabolic syndrome (MS) in different age groups of Tianjin policemen. Methods: Based on the clinical data of periodic heath examination, 1 250 policemen were divided into MS group and non- MS (Nms) group. The levels of liver function, renal function, lipid, heart rate (HR), white blood cell (WBC), platelet (PLT), fibrinogen (FIB) and blood sedimentation were analyzed in two groups. The risk factors of MS were analysed between policemen ≥40 years old and policemen <40 years old. Results: The levels of alanine transaminase (ALT), alkaline phospha-tase(ALP), γ-glutamyl transferase (γ-GT), total protein(TP), albumin(ALB), globulin(GLB), uric acid (UA), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), HR, WBC and FIB were significantly higher in MS group than those of non-MS group. But the level of high-density lipoprotein cholesterol (HDL-C) was significantly lower in MS group than that of non-MS group (P <0.01). The risk factors of MS in policemen under the age of 40 were the increased serum levels of ALT, blood UA and FIB (OR = 2.056, 95%CI 1.125-3.759) and HR. While the risk factors of MS in policemen beyond the age of 40 included serum TP, blood UA, γ-GT and increased WBC and HR. Conclusion: Hyperfibrinogenemia is the high risk factor for MS in policemen under the age of 40.
