早发严重冠状动脉三支病变患者冠心病危险因素的分布及聚集

摘要

Objective :To investigate the distribution of known risk factors of coronary heart disease (CHD) for premature severe three-vessel coronary artery lesions. Methods: Clinical data from 113 patients with severe three-vessel coronary artery lesions(CHD group) and 57 patients with normal coronary artery(control group) were collected in this study. The distribution of risk factors were analyzed. Results: (1)There were significant differences in gender, smoking bistory, triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), body mass index (BMI), essential hypertension (EH), diabetes mellitus (DM) and positive family history(PFH) of CHD between groups (P＜0.05 or P＜0.01). Multiple logistic regression analysis showed thai risk factors for premature three-vessel coronary lesions were male, PFH, DM and hypertriglyceridemia.(2)Compared with the control group, the degree of aggregation significantly increased in 3 or more risk factors in CHD group.(3)Subgroup analysis indicated that smoking, PFH, BMI and HDL-C were significantly different between male patients of two groups (P＜0.05 or P＜0.01). And there were significant differences in DM, EH, TG and HDL-C between female patients of two groups (P＜0.05 or P＜0.01). Conclusion:The risk reactors for premature severe three-vessel coronary artery lesions were male, PFH, DM and hypertriglyeeridemia, and the aggregation of the main risk factors existed. Risk factors were remarkably different between men and women.%目的:探讨早发严重冠脉三支病变患者已知冠心病(CHD)危险因素的分布及聚集.方法:对比早发严重冠脉三支病变患者(CHD组)113例与冠脉正常患者(对照组)57例的临床资料,筛选早发严重冠脉三支病变的危险因素并分析其聚集情况.结果:(1)2组间性别、吸烟史、三酸甘油(TG),高密度脂蛋白胆固醇(H DL-C)、体质量指数(BMI)、高血压病(EH)、糖尿病(DM),CHD家族史差异有统计学意义(P<0.05或P<0.01).多元Logistic回归分析显示早发严重冠脉三支病变的危险因素有男性、CHD家族史、DM和高TG血症.(2)CHD组较对照组3种以上危险因素聚集程度明显增加.(3)按性别行亚组分析显示男性CHD与对照2组间吸烟史、CHD家族史、BMI、HDL-C差异有统计学意义(P<0.05),女性CHD与对照2组间DM、EH、TG、HDL-C差异有统计学意义(P<0.05或P<0.01).结论:男性、CHD家族史、DM、高TG血症是早发严重冠脉三支病变的危险因素,且此类患者存在多种危险因素聚集现象.男女间CHD危险因素存在差异.
