一般健康人群中白介素-6和超敏C-反应蛋白与胰岛素抵抗的相关性比较

         

摘要

目的 比较白介素-6(IL-6)和超敏C-反应蛋白(hs-CRP)与胰岛素抵抗(IR)的相关性.方法 纳入154例一般健康人群,测定体重、身高、血压、空腹血浆葡萄糖(FPG)、血清胰岛素(Fins)、血脂、IL-6和hs-CRP等,计算体重指数(BMI)和IR指数(HOMA-IR),并以HOMA-IR上四分位为切点分为IR组与非IR组.结果 IR人群血清IL-6和hs-CRP水平较非IR人群明显升高(P<0.05).Pearson相关分析显示,IL-6和hs-CRP均与HOMA-IR相关(r=0.58,r =0.48,P<0.05).校正年龄、BMI后,IL-6仍然与HOMA-IR密切相关(r=0.14,P<0.05),而hs-CRP与HOMA-IR的相关性无统计学意义(r =0.06,P>0.05).进一步作Logistic回归分析发现,高IL-6人群发生IR的风险较低IL-6人群增加2.46倍[OR(95% CI):2.46(1.48~3.39),P<0.05],而高hs-CRP人群发生IR的风险较低CRP人群仅增加1.82倍[OR(95%CI):1.82(1.14 ~2.67),P<0.05].结论 在一般健康人群中,与hs-CRP相比,IL-6是更好的IR预测指标.%Objective To compare the associations of Interleukin 6 (IL-6) and high sensitive C-reactive protein (hs-CRP) with insulin resistance (IR) in general healthy subjects. Methods 154 general healthy subjects were recruited. Weight, height, blood pressure, fasting plasma glucose (FPG) , fasting serum insulin (Fins) , blood lipids, IL-6 and hs-CRP were measured. Body mass index ( BMI) and HOMA index ( HOMA-IR) was calculated. IR was defined as the upper quartile of HO-MA-IR. Results Serum IL-6 and hs-CRP were significantly higher in subjects with IR compared with subjects without IR (P < 0. 05). Spearman correlation analysis showed that both IL-6 and hs-CRP positively correlated with H0MA-IR( r = 0. 58 ,r = 0. 48, P<0. 05). Partial correlation analysis with control of BMI showed that PEDF was positively correlated with hs-CRP. After adjustment for age, and BMI, IL-6 was still associated with HOMA-IR (r=0. 14, P < 0. 05); However, the association between hs-CRP and HOMA-IR no longer existed. Moreover, Logistic regression analysis revealed that compared to the subjects with low IL-6, those with high IL-6 were 2.46[OR(95%CI):2.46(1.48~3. 39), P<0.05] times more likely to exhibit IR; while compared to the subjects with low hs-CRP, those with high hs-CRP were just 1.82 [OR(95%CI) : 1. 82( I. 14~2.67), P<0. 05] times more likely to be exhibit IR. Conclusion compared with hs-CRP, IL-6 might be a better marker of IR in general healthy subjects.
