磁化转移成像技术对于视神经脊髓炎患者脑组织的研究

             

摘要

Objective To study whether the brain tissue of the patients with neuromyelitisoptica( NMO) in its early stage has occult change by magnetization of transfer imaging(MTI). Methods Data of 27 cases of patients clinically diagnosed with NMO were collected including course of disease,age,main clinical symptoms and EDSS score. 27 cases of those patients and 30 ca-ses of normal volunteers who matched with those patients in age and sex were scanned with 3. 0T magnetic resonance technology. Then we analyzed patients’ ROI(regions of interest)in grey matter(bilateral insular lobe,bilateral caudate nucleus,bilateral cau-date nucleus,bilateral white ball,bilateral thalamus and bilateral cingulate gyrus) and the ROI in white matter( genu and splenium of corpus callosum, bilateral cerebral peduncle, bilateral posterior limb of internal capsule and bilateral posterior coronal radia-tion). Results Compared with the control group,the acute NMO group had a higher magnetization transfer ratio(MTR)in right bean nucleus,bilateral white ball and bilateral cingulate gyrus,and a lower MTR bilateral cerebral peduncle. The MTR of right caudate nu-cleus,bilateral white ball and bilateral cingulate gyrus in stable phase NMO group was lower than that of control group. There was no difference in other ROI(P<0. 05). Conclusion The difference of MTR in grey and white matter of patients with NMO displayed on the MTI indicated a occult injury which might was different in different stage.%目的:应用磁化转移成像技术( Magnetization Transfer Imaging,MTI)研究视神经脊髓炎( Neuromyelitis Opti-ca,NMO)患者早期脑组织是否存在隐匿性改变。方法收集27例临床确诊为NMO患者的临床资料(包括病程、年龄、主要临床症状、EDSS评分)。采用3.0T磁共振技术对27例NMO患者进行头部MTI扫描,同时对30例年龄、性别相匹配的正常自愿者进行相同序列的扫描。对患者的灰白质感兴趣区(包括双侧岛叶、双侧尾状核、双侧豆状核、双侧苍白球、双侧丘脑、双侧扣带回)和白质感兴趣区(包括胼胝体膝部、胼胝体压部、双侧大脑脚、双侧内囊后肢、双侧放射冠后部)进行分析。结果急性期NMO组右侧豆状核、双侧苍白球、双侧扣带回的磁化传递率( MTR)值较正常健康对照组的MTR值升高。稳定期NMO组右侧尾状核、双侧苍白球、双侧扣带回的MTR值较正常健康对照组的MTR值降低。急性期NMO组双侧大脑脚的MTR值较健康对照组降低。其余的感兴趣区灰白质部位与健康对照组MTR比较差异无统计学意义( P>0.05)。结论 NMO患者大脑灰白质磁化转移成像扫描显示MTR存在差异提示可能存在隐匿性损伤,这种隐匿性改变在急性期与稳定期可能存在差异。
