硬膜外镇痛泵联合鸡尾酒关节注射在膝关节置换术中的应用

摘要

目的 探讨硬膜外自控镇痛泵联合鸡尾酒关节注射在人工全膝关节置换术(TKA)的应用效果.方法 将185例行单侧T KA患者分为对照组和观察组.对照组应用硬膜外自控镇痛泵,观察组在此基础上行鸡尾酒关节注射镇痛.比较2组镇痛效果、术后膝关节功能和不良反应发生情况.结果 观察组术后48 h内的静息和被动运动状态疼痛视觉模拟评分(VAS)均低于对照组(P＜0 .05) ;观察组术后3个月内的膝关节学会评分系统(KSS)评分高于对照组(P＜0 .05) ;2组不良反应发生率差异无统计学意义(P＞0 .05).结论 硬膜外自控镇痛泵联合鸡尾酒关节注射用于KTA的术后镇痛效果明显,且利于术后膝关节功能的恢复.%Objective To discuss the application effect of epidural controlled analgesia pump combined with cocktail joint injections in knee arthroplasty .Methods One hundred and eighty-five patients with unilateral TKA were divided into control group and observation group .In the control group ,an epidural analgesic pump was used , and the observation group took a cocktail injection of analgesia .The effects of analgesia ,postoperative knee func-tion and adverse reactions were compared between the two groups .Results The VAS scores of resting and pas-sive exercise in the observation group within 48 hours after operation were lower than those in the control group (P＜0.05).The KSS score in the observation group within 3 months after operation was higher than that in the control group (P＜0.05).There was no significant difference in the incidence of adverse reactions between the two groups (P＞ 0.05).Conclusion The postoperative analgesic effect of epidural patient-controlled analgesia pump combined with cocktail joint injection for KTA is obvious ,and is beneficial to the recovery of postoperative knee joint function .
