“老年护理病房-居家护理”一体式模式的效果探讨

摘要

Objective: To study how to apply the nursing service mode integrated with the elderly nursing ward and home in the community health service center, and to evaluate its effect in order to provide the full, comprehensive and seamless extension services for elderly patients.Methods:FromJuly 2014 to June 2015, 44 elderly patients discharged from the elderly nursing words of Jiading Community Health Service Center were selected and divided into an observation group with 23 cases and a control group with 21 cases. The control group was given the conventional discharge guidance, on the basis of the conventional discharge guidance, the observation group was given the home nursing service, and the effects of extended nursing on elderly patients after being discharged were assessed. The results of the patients’ life self-care ability scale, their satisfaction, diseases awareness and re-hospitalization rate were compared between the patients in the two groups.Results: The scores of the self-care ability scale, satisfaction and diseases awareness and re-hospitalization rate were better in the observation group than those in the control group.Conclusion: The nursing service mode integrated with the elderly nursing ward and home makes the patients get the effective cohesion and extension services so as to improve their satisfaction, increase the turnover rate of beds, and alleviate the social problem that it is hard to ifnd a bed in the nursing institutes.%目的：探讨“老年护理病房—居家护理”一体式护理服务模式如何在社区卫生服务中心应用，并对其进行效果评价，以期为老年患者提供全程、全面、无缝的延伸服务。方法：选取2014年7月—2015年6月在嘉定镇街道社区卫生服务中心老年护理病房的出院老年患者44例，分为观察组23例和对照组21例，对照组给予常规的出院指导，观察组在常规出院指导基础上，实施居家护理，评估老年患者出院后经过居家延伸护理后的效果，比较两组患者生活自理能力量表的情况，以及患者满意度、疾病知晓率和再住院率的情况。结果：观察组自理能力量表评分、满意度、疾病知晓率及再住院率明显优于对照组。结论：“老年护理病房—居家护理”一体式护理服务模式使患者的护理得到了有效的衔接和延伸，提高了患者满意度，增加了床位周转率，缓解了一床难求的社会问题。
