同轴微切口白内障超声乳化吸除术对角膜散光影响研究

             

摘要

目的:观察和比较1.8m m透明角膜微切口与3.2m m透明角膜小切口白内障超声乳化吸除术后对角膜散光的影响。方法:选取行白内障超声乳化人工晶体植入术的年龄相关性白内障患者85例117眼,将患者按随机数字表法分为两组。微切口组59眼行1.8 m m同轴白内障超声乳化吸除术,小切口组58眼行3.2m m同轴白内障超声乳化吸除术。术前、术后1d、术后7d、术后30d和术后90d分别进行LogMAR视力、中央角膜厚度和角膜散光检查。比较两组术前和术后不同时间点各项检查结果的差异。结果:术后1d微切口组比小切口组视力恢复快,差异具有统计学意义(P<0.05),余时间点两组间视力差异无统计学意义(P>0.05)。术后各时间点微切口组和小切口组的中央角膜厚度增厚差异无统计学意义(P>0.05)。术后各时间点微切口组和小切口组的手术源性散光差异有统计学意义( P>0.05),两组均在术后30d角膜散光趋于稳定。结论:与3.2mm小切口白内障超声乳化吸除术相比,同轴1.8mm微切口白内障超声乳化吸除术的手术源性散光小,术后视力恢复快。%Objective:To observe and compare changes in corneal astigmatism in patients after clear cor-neal incision phacoemulsification with 1 .8mm and 3 .2mm .Methods:Using a random number table ,117 eyes of 85 patients with age-related cataract in our hospital were divided into two groups:59 eyes in the coaxial 1 .8mm microincision cataract surgery group(C-MICS) ,58 eyes in the coaxial 3 .2mm traditional small incision cataract sur-gery group(C-SICS) .117 eyes were received phacoemulsification cataract surgery with intraocular lens implan-tation .Uncorrected visual acuity ,central corneal thickness and corneal astigmatism were recorded preoperatively and postoperatively at 1 ,7 ,30 and 90d .Results :At 1d postoperative ,Uncorrected visual acuity results of C-MICS were better than the results of C-SICS .The difference was statistically significant(P<0 .05) .There were no significance on central corneal thickness increase between C-MICS and C-SICS at1 ,7 ,30 and 90d after surgery(P<0 .05) .Com-pared with C-SICS ,surgically-induced astigmatism in C-MICS was smaller at 1 ,7 ,30 and 90d after surgery .The difference was statistically significant (P<0 .05) .Conclusion:Compared with 3 .2mm tradional small incision ,coaxial 1 .8mm microincision phacoemulsification is associated with less surgically-induced astigmatism and could get earlier visual rehabilitation postoperatively .
