老年慢性心力衰竭患者外周血甲状旁腺激素水平与心功能的关系研究

摘要

Objective:To analyze the elderly patients with chronic heart failure peripheral blood parathyroid hormone (PTH) level and cardiac function,so as to provide reference for clinical diagnosis and treatment.Methods:The clinical data of elderly patients with chronic heart failure.The median serum PTH level in patients with pg/ml (99.6.Chemiluminescence) patients were further divided into observation group (PTH =99.6,pg/ml) and control group (PTH < 99.6 pg/ml).Compared two groups of cardiac function and peripheral blood C-reactive protein (CR P),Cystatin C (Cys-C) and interleukin-6 (IL-6) level,analysis of the relationship between PTH and heart function grade.The results included 39 cases in the observation group,41 cases in the control group.The patients in the observation group LVEF,6min walking distance and NYHA I-II ratio were significantly lower than those in the control group,all have statistical difference (P<0.05).To observe the peripheral blood of patients in group CRP,Cys-C and IL-6 were significantly higher than those in control group,compared the difference was statistically significant (P<0.05).Serum PTH level and NYHA in elderly patients with chronic heart failure there was a significant positive correlation between the levels (r=0.662,P<0.05),which was statistically significant.Conclusion:The increase of PTH in peripheral blood of patients with chronic heart failure (CHF) indicates that the heart function is not good,which can be used to evaluate the cardiac function.%目的:分析老年慢性心力衰竭患者外周血甲状旁腺激素(PTH)水平与心功能的关系,为临床诊疗提供参考.方法:分析老年慢性心力衰竭患者的临床资料.以患者血清PTH水平的中位数(99.6 pg/ml,化学发光法)为界将患者分为观察组(PTH≥99.6 pg/ml)及对照组(PTH<99.6 pg/ml).对比两组心功能及外周血C-反应蛋白(CRP)、胱抑素C(Cys-C)及白介素-6(IL-6)水平,分析PTH与心功能等级的关系.结果:本项目共纳入观察组39例,对照组41例.观察组患者左室射血分数(LVEF)、6 min步行距离及NYHAⅠ-Ⅱ级比例相较对照组更低,差异有统计学意义(P<0.05).观察组外周血CRP、Cys-C及IL-6指标高于对照组,差异有统计学意义(P<0.05).老年慢性心力衰竭患者外周血PTH水平与NYHA等级呈现显著正相关关系(r=0.662,P<0.05),具有统计学意义. 结论:老年慢性心力衰竭患者外周血PTH水平升高提示心脏功能不良,可用于临床辅助评估心功能.
