法舒地尔联合尼莫地平对颅内宽颈动脉瘤栓塞术后脑血管痉挛患者脑供血及神经功能影响研究

摘要

目的:探讨法舒地尔联合尼莫地平对颅内宽颈动脉瘤栓塞术后脑血管痉挛患者脑供血及神经功能的影响.方法:选取颅内宽颈动脉瘤栓塞术后脑血管痉挛患者160例,随机分为观察组和对照组,各80例.对照组患者给予尼莫地平进行治疗,观察组患者在此基础上加用法舒地尔进行治疗.观察两组患者格拉斯哥预后量表(GOS)评分、大脑中动脉(MCA)平均血流速度以及神经功能缺损(NFI)评分和巴氏(Barthel)指数评分并进行比较分析.结果:术后观察组GOS评分5分的比例为57.50%,对照组为25.00%,差异有统计学意义(P<0.05).两组治疗后大脑中动脉平均血流速度显著升高,观察组升高更显著,差异有统计学意义(P<0.05).与治疗前相比,治疗后NFI评分均显著降低,且观察组降低更显著;治疗后Barthel评分均显著高于治疗前,且观察组升高更明显,差异有统计学意义(P<0.05).结论:法舒地尔联合尼莫地平治疗颅内宽颈动脉瘤患者手术后脑血管痉挛效果显著,可增加脑血管供血,有助于神经功能恢复,提高患者生活质量.%Objective:To investigate the effect of fasudil combined with nimodipine on intracranial aneurysms after embolization of cerebral vascular spasm in patients with blood supply and nerve function.Methods:160 patients with cerebral vasospasm after wide necked aneurysm were selected and randomly divided into observation group and control group,80 cases in each group.The control group were treated with nimodipine treatment,patients in the ob-servation group on the basis of usage fasudil treatment.The Glasgow Outcome Scale(GOS)score,middle cerebral artery(MCA)mean blood flow velocity,nerve function defect(NFI)score and Barthel index score were observed and compared between the two groups.Results:The proportion of GOS score of 5 in the observation group was 57. 50%,and the control group was 25%,and the difference was statistically significant(P<0.05).The average veloc-ity of blood flow in the middle cerebral artery increased significantly in the two groups,and the increase of the obser-vation group was more significant(P<0.05).Compared with before treatment,NFI scores were significantly de-creased after treatment,and the observation group decreased more significantly.After treatment,Barthel scores were significantly higher than those before treatment,and the observation group increased more significantly,the difference was statistically significant(P<0.05).Conclusion:Fasudilcombined with nimodipine in the treatment of wide necked intracranial artery aneurysm surgery in patients with cerebral vascular spasm effect significantly,can in-crease the brain blood supply,contribute to the recovery of neurological function,improve the quality of life of pa-tients.
