目的 探讨醋酸去氨加压素结合术中护理对鞍区肿瘤术后尿崩症的干预治疗效果. 方法 回顾性分析我院神经外科2010年1月至2014年12月收治的1 542例鞍区肿瘤患者的手术资料和护理资料. 根据患者资料的完整性、实验分析的有效性,1 130例患者符合我们的实验设计要求,其中对照组384例(术前、术中未经过任何干预) ,单一干预组381例(仅术前给予醋酸去氨加压素) ,联合干预组365 例(术前给予醋酸去氨加压素联合术中护理),术后以患者尿量、尿比重、尿渗透压、尿色等作为尿崩症诊断指标. 结果 对照组患者术后出现尿崩症305 例,尿崩症的发生率为79. 43%;单一干预组患者术后出现尿崩症133 例,尿崩症的发生率为34. 91%;联合干预组患者术后出现尿崩症21例,尿崩症的发生率为5. 75%. 单一干预组与对照组相比,尿崩症的发生率明显降低,差异有统计学意义(P0. 05). 结论 醋酸去氨加压素结合术中护理能够有效预防鞍区肿瘤术后尿崩症的发生.%Objective To investigate the effect of the combination of desmopressin with intraoperative nursing on postoperative diabetes insipidus in sellar tumors. Methods The surgical nursing data and clinical information of 1 542 cases of sellar tumors in neurosurgery department of our hospital from January 2010 to December 2014 were ana-lyzed retrospectively. Based on the integrity of cases data and the effectiveness of experimental analysis,1 130 patients were divided into three groups. Patients in single control group (n=384)were given no preoperative and intraoperative intervention,patients in single intervention group (n=381)were given only preoperative desmopressin,and patients in joint intervention group (n=365)recieved preopertive desmopressin combined with intra-operative care. Diabetes insip-idus was diagnosed according to postoperative urine volume, specific gravity, osmotic pressure and color. Results There were 305 cases of postoperative diabetes insipidus (79. 43%)in control group,133 cases (34. 91%)in single in-tervention group,21 cases (5. 75%) in joint intervention group. The incidence of diabetes insipidus of single interven-tion group was lower than that of control group(P0. 05 ) . Conclusion Desmopressin com-bined with intraoperative nursing could effectively prevent the occurrence of diabetes insipidus after sellar tumor surgery.
