万古霉素局部应用在预防假体关节感染中的有效性及安全性研究

摘要

目的 探讨局部应用万古霉素粉针剂在预防假体关节感染(PJI)中的有效性及安全性.方法 回顾性分析2010年1月至2014年12月在我院接受关节置换的患者387例,其中万古霉素组(治疗组)249例,普通方案组(对照组)138例.比较两组患者PJI发生率、不良反应发生率,以及接受单侧关节置换与双侧关节置换患者的PJI发生率.结果 治疗组PJI发生率显著低于对照组(P＜0.05),不良反应发生率略高于对照组,但差异无统计学意义(P＞0.05);单侧关节置换与双侧关节置换患者的PJI发生率比较差异无统计学意义(P＞0.05).结论 局部应用万古霉素粉针剂可降低关节置换患者术后PⅡ的发生率,且不良反应发生率较低.%Objective To investigate the efficacy and safety of local application of vancomycin powder in the prevention of prosthetic joint infection (PJI).Methods A retrospective analysis of 387 patients undergoing joint replacement in our hospital from January 2010 to December 2014 was performed;there were 249 cases in vancomycin group (treatment group) and 138 cases in general program group (control group).The incidence of PJI,adverse reactions,and PJI in patients undergoing unilateral and bilateral joint replacement were compared between the two groups.Results The incidence of PJI in treatment group was significantly lower than that of control group (P ＜ 0.05),and the incidence of adverse reactions was slightly higher than that of control group without significant difference between the two groups (P ＞ 0.05).There was no significant difference in the incidence of PJI between the patients with unilateral joint replacement and bilateral joint replacement (P ＞ 0.05).Conclusion Local application of vancomycin powder injection can reduce the incidence of PJI in patients undergoing joint replacement,and the incidence of adverse reactions is low.
