颅脑损伤患者血清胶质纤维酸性蛋白含量变化及其临床意义

         

摘要

目的 探讨颅脑损伤患者血清中胶质纤维酸性蛋白(GFAP)的变化及其临床意义.方法 选择2008年6月至2010年5月在我院住院治疗的97例颅脑损伤患者(观察组)及同期30例健康体检者(对照组),采用酶联免疫吸附法(ELISA)检测两组血清GFAP、S-100B和神经元特异性烯醇化酶(NSE)水平,分析其与格拉斯哥昏迷评分及预后评分的关系.结果 颅脑损伤患者在伤后12h血清GFAP水平显著高于对照组(P<0.05);血清GFAP水平与48 h的格拉斯哥昏迷评分及6个月后格拉斯哥预后评分呈负相关(P<0.05);应用ROC曲线分析,血清GFAP曲线下面积显著大于S-100B和NSE(P<0.05).结论 血清GFAP检测可作为判断颅脑损伤早期病情的指标之一,其敏感性优于常规指标S-100B和NSE.%Objective To investigate the changes and clinical significance of serum glial fibrillary acidic protein (GFAP) in patients with craniocerebral injury.Methods From June 2008 to May 2010,97 cases of craniocerebral injury patients were hospitalized and selected as observation group and 30 healthy subjects as control group.Serum levels of GFAP,S-100B and neuron specific enolase (NSE) level were detected by using enzyme-linked immunosorbent assay (ELISA).The relationship between GFAP and Glasgow coma score and prognosis was analyzed.Results The serum level of GFAP in the patients with traumatic brain injury after 12 hr of injury was significantly higher than that in the control group (P < 0.05).The level of serum GFAP was negatively correlated with 48 hour Glasgow coma score and 6 month Glasgow outcome score (P < 0.05).The ROC curve analyses showed that the area under the curve of serum GFAP was significantly larger than that of S-100B or NSE (P < 0.05).Conclusion The detection of serum GFAP can be used.as one of the indicators to judge the early stage of brain injury.Its sensitivity is better than that of conventional S-100B and NSE.
