超声弹性成像鉴别非哺乳期乳腺炎和乳腺癌的临床价值

摘要

目的 分析超声弹性成像鉴别非哺乳期乳腺炎和乳腺癌的临床应用价值.方法 回顾性分析55例非哺乳期乳腺炎和55例乳腺癌患者的超声弹性成像,并评估其对两者的鉴别价值.结果 与乳腺炎患者相比较,乳腺癌患者后方回声衰减、钙化、Ⅱ～Ⅲ级内部血流、RI ≥0.70以及UE 4 ～5分所占比例显著增高,组间差异具统计学意义(P＜0.05).乳腺癌组病灶硬度高分所占比例显著高于乳腺炎组,组间差异具统计学意义(P＜0.05).常规超声诊断乳腺炎和乳腺癌符合率为85.5％和78.9％,超声弹性成像诊断乳腺炎和乳腺癌符合率为90.9％和91.2％,两者差异具统计学意义(P＜0.05).常规超声诊断敏感度、特异度以及准确率分别为78.9％、85.5％和82.1％,超声弹性成像诊断敏感度、特异度以及准确率分别为90.9％、91.2％和91.1％,两者差异具统计学意义(P＜0.05).结论 超声弹性成像可有效鉴别乳腺肿块的良恶性,准确率高,临床价值肯定.%Objective To analyze the clinical value of ultrasound elastography in differential diagnosis of non-lactation mastitis and breast cancer.Methods 55 cases of non-lactation mastitis and 55 cases of breast cancer were collected,and a retrospective analysis of diagnostic value of ultrasonic elastography were carried out.Results Compared with the mastitis group,the breast cancer patients had significantly higher proportions in the following indices:posterior echo attenuation,caloification,Ⅱ ～ Ⅲ internal blood flow,RI ≥ 0.70,and UE 4 ～ 5,with statistically significant difference (P ＜ 0.05).The proportion of high hardness in the breast cancer group was significantly higher than that of the mastitis group,with statistically significant difference (P ＜ 0.05).The conventional ultrasound had diagnostic coincidence rates of 85.5％ and 78.9％ in mastitis and breast cancer respectively.In contrast,ultrasonic elastography had the diagnostic coincidence rate of 90.9％ and 91.2％ in the mastitis and breast cancer,with statistically significant difference (P ＜ 0.05).The diagnostic sensitivity,specificity and accuracy rate of conventional ultrasound were 78.9％,85.5％ and 82.1％ respectively,while those of ultrasonic elastography were 90.9％,91.2％ and 91.1％,with statistically significant difference (P ＜ 0.05).Conclusion Ultrasound elasticity imaging can effectively identify benign and malignant breast masses;the accuracy rate is high,and the clinical value is positive.
