By participating in the treatment for an old patient with cardiovascular disease, clinical phar-macists made proper suggestions according to the genotype, drug interaction, disease, etc, and assisted physicians in making individualized antiplatelet treatment plans. Clinical pharmacists should provide pharma-ceutical care in the whole course, and guarantee the safety, efficacy and economical efficiency of drug use.%临床药师通过参与1例老年心血管疾病患者的治疗过程，根据患者基因型、药物相互作用等因素，协助医师制订个体化抗血小板用药方案；临床药师对患者整个用药过程进行监护，保障患者用药安全、有效、经济。
