Objective To investigate the current situation of drug withdrawal in our in-hospital pharmacy,and to analyze the causes, in order to provide the effective arrangement for improving the rational drug use and hospital management level. Methods Statistical analysis on the 5 962 cases application of the drug withdrawal in our in-hospital pharmacy was carried out,and rational suggestions was put forward. Results Total drug withdrawal proportion was 10. 55% ,among which department of pediatrics occupied the largest amount(17. 15% in total). The main category of accidents of drug withdrawal were as follows; patients were discharged from hospital,doctors changed orders, the prescription mistook, and so on. Conclusion We should strengthen the drug management, improve the sense of responsibility and professional qualities, and focus on perfecting information system, in order to guarantee the drug use safety.%目的 调查医院住院药房退药情况,分析退药原因,采取有效的防范措施,促进医院合理用药,提高药房管理水平.方法 收集我院住院药房5 962例退药申请单,从退药科室、退药原因、种类、金额进行统计分析并提出建议.结果 总退药比例为10.55%,退药量最大的科室为儿科,所占比例也最大,为17.15%,存在退药的药品比例最大的是抗菌药物,主要退药原因有出院、变更医嘱、处方错误等.结论 强化医生合理用药意识,加强药品管理,进一步完善信息系统,有效控制退药,保证临床用药安全.
