银杏叶提取物联合糖皮质激素治疗特发性肺纤维化疗效及对血清TGF-β1、IL-13、SOD、MDA的影响

     

摘要

Objective It is to observe the effect of Ginkgo biloba extract combined with glucocorticoids in the treatment of idiopathic pulmonary fibrosis (IPF) and its influence on serum transforming growth factor beta1 (TGF-β1) and interleukin-13 (IL-13), serum superoxide dismutase (SOD), malondialdehyde (MDA) level. Methods 86 patients with IPF were randomly divided into the observation group (43 cases) and the control group (43 cases), the patients was treated with glucocorticoid in the control group and with Ginkgo biloba extract combined with glucocorticoids in the observation group, the course of treatment was 12 weeks. The changes of blood gas analysis, lung function and lung fiber index were observed before and after treatment in the two groups; the levels of serum TGF-β1, IL-13, SOD and MDA were measured before and after treatment in two groups. Results The levels of p(O2) and Sa(O2) were significantly higher and the levels of p(CO2) and p(A-a)(O2) were significantly lower of the two groups after treatment (all P<0.05), but the improvement of p(O2) and p(A-a)(O2) in the observation group was better than that in the control group (P<0.05); FVC, FEV1/FVC, MVV and DLCO were increased significantly in the two groups (all P<0.05), the improvement of the indexes in the observation group was obviously better than that in the control group (all P<0.05); the PC Ⅲ, C Ⅳ, PICP and LN levels were decreased obviously in the two groups after treatment (all P<0.05), the improvement of the indexes in the observation group was obviously better than that in the control group (all P<0.05); the levels of serum TGF-β1, IL-13 and MDA were significantly lower in the two groups (all P<0.05), the levels of SOD were significantly higher (P<0.05), the improvement of the indexes in the observation group was better than that in the control group (P<0.05). Conclusion Ginkgo biloba extract combined with glucocorticoids can significantly improve arterial blood gas index and lung function in patients with IPF, and block the process of pulmonary fibrosis, it may be related to the decrease of serum TGF-β1, IL-13, MDA levels, and the increase of SOD activity.%目的 观察银杏叶提取物联合糖皮质激素治疗特发性肺纤维化(IPF)的疗效及对血清转化生长因子-β1(TGF-β1)、白细胞介素-13(IL-13)、血清超氧化物歧化酶(SOD)、丙二醛(MDA)水平的影响.方法 将86例IPF患者随机分为观察组43例及对照组43例,对照组给予糖皮质激素治疗,观察组在常规治疗基础上给予银杏叶提取物治疗,疗程12周.观察2组治疗前后血气分析、肺功能、肺纤维指标变化情况;检测2组治疗前后血清TGF-β1、IL-13、SOD、MDA水平.结果 2组治疗后p(O2)、Sa(O2)均明显升高(P均<0.05),p(CO2)、p(A-a)(O2)均明显降低(P均<0.05),且观察组p(O2)、p(A-a)(O2)改善情况均优于对照组(P均<0.05);2组治疗后FVC、FEV1/FVC、MVV、DLCO均明显升高(P均<0.05),且观察组上述指标改善情况均明显优于对照组(P均<0.05);2组治疗后PCⅢ、CⅣ、PICP、LN均明显降低(P均<0.05),且观察组上述指标改善情况均明显优于对照组(P均<0.05);2组治疗后血清TGF-β1、IL-13、MDA水平均明显降低(P均<0.05),SOD水平均明显升高(P均<0.05),且观察组上述指标改善情况均优于对照组(P均<0.05).结论 银杏叶提取物联合糖皮质激素能够显著改善IPF患者动脉血气指标及肺功能,并阻断肺纤维化进程,可能与降低血清TGF-β1、IL-13、MDA水平,提高SOD活性有关.
