彩色多普勒超声引导下经皮经肝胆管穿刺置管引流术40例分析体会

摘要

Objective To explore the operation method and experienceof percutaneous transhepatic cholangial drainage ( PTCD) guided by color Doppler ultrasound. Methods 40 cases of patients with obstructive jaundiceinduced by malignant tumor, bile duct stricture, bile duct stones and othercauses from February 2013 to June 2015 were performed with PTCD guided by color Doppler ultrasound. Results 40 cases of patients were successfully puncture and drainage. There were 2 cases of patients performed with left and right bile duct intubation. No cases of patients occured with serious complications. Conclusion PTCD is an effective,method for treatment of obstructive jaundice. The puncture equipment is simple, safety, high accuracy, real⁃time monitoring, dynamic monitoring of the whole, traumatic is small and may be carried out by the bedside.%目的：探讨彩色多普勒超声引导下经皮经肝胆管穿刺置管引流术（ percutaneous transhepatic cholangial drainage， PTCD）的操作方法及穿刺体会。方法2013年2月－2015年6月，在彩色多普勒超声引导下对临床及影像学证实的40例恶性肿瘤、胆道狭窄、胆道结石等原因导致的梗阻性黄疸患者，进行经皮经肝胆管置管引流术。结果40例均穿刺置管引流成功，其中2例行左右胆管同时置管；无相关严重并发症。结论 PTCD是一种治疗梗阻性黄疸的有效方法，穿刺设备简单，并可实时动态全程监测，可床边进行，便捷安全，准确性高，创伤性小。
