前列地尔的长程治疗对糖尿病肾病患者肾功能及尿蛋白的影响

     

摘要

Objective To observe the effect of long-term alprostadil on renal function and proteinuria a-mong the patients with diabetic nephropathy. Methods Totol of 44 patients with diabetic nephropathy trea-ted in Changshu Chinese medicine hospital from July 2012 to June 2013 was selected. The patients were ran-domly divided into alprostadil group (19 cases) and the conventional treatment group (25 cases). Conven-tional treatment group was treated by conventional therapy, alprostadil group was treated by intravenous infu-sion of alprostadil per day for 28 consecutive days on the basis of foundation treatment. Changes of the renal function and proteinuria of 0, 7, 14, 21 and 28 days were observed,compared the adverse reaction. Results The fasting plasma glucose between the two groups on interblock,different time points and interblock and time points had no statistically significance(P>0. 05). The urea nitrogen,creatinine and proteinuria between two groups in interblock, different time points and interblock and time points had statistically significance( P<0. 05),compared with conventional treatment group, the urea nitrogen,creatinine and proteinuria of al-prostadil group were decsreased,and with the extension of time,they decreased obviously (P<0. 05). There was no adverse reaction between the two groups. Conclusion Long-term alprostadil therapy can effective re-ducing the levels of urea nitrogen,creatinine and proteinuria. There were no significant adverse reactions oc-curred in the course of the treatment.%目的:观察前列地尔的长程治疗对糖尿病肾病( DN)患者肾功能及尿蛋白的影响。方法选取2012年7月至2013年6月在常熟市中医院治疗的DN患者44例作为研究对象,按完全随机化的方法将患者分为前列地尔组(19例)和常规治疗组(25例)。常规治疗组患者予以常规治疗,前列地尔组在常规治疗的基础上加用前列地尔静脉注射治疗,每日1次,连续28 d。观察两组患者治疗0、7、14、21、28 d后肾功能及尿蛋白的变化以及不良反应的发生情况。结果两组患者的空腹血糖在组间、不同时间点及组间·不同时间点比较,差异均无统计学意义(P>0.05);两组患者尿素氮、肌酐及蛋白尿在组间、不同时间点及组间·不同时间点比较,均具有统计学意义(P<0.05),与常规治疗组相比,前列地尔组患者的尿蛋白、尿素氮及肌酐水平显著下降,且随着时间的延长,降低的更为显著(P<0.05);两组患者均未发生明显的不良反应。结论前列地尔的长程治疗可有效减少尿素氮、肌酐及蛋白尿的水平,保护肾功能,且治疗过程中并未出现明显的不良反应。
