血清 Hcy及NT-proBNP水平与缺血性脑卒中患者神经功能缺损及脑梗死体积的相关性

             

摘要

Objective To investigate correlation between serum homocysteine (Hcy),N-terminal pro-brain natriuretic peptide(NT-proBNP) levels and National Institute of Health Stroke Scale (NIHSS) score, cerebral infarct volume in patients with ischemic stroke .Methods Total of 100 patients with cerebral arterial thrombosis admitted to Yulin First Hospital from Mar.2014 to Mar.2015 were included as a stroke group, another 100 health people during the same period were included as a control group .The levels of serum Hcy and NT-proBNP were compared between the two groups,and the correlation of the levels of serum NT-proBNP and Hcy with NIHSS and cerebral infarct volume was analyzed .Results The serum levels of Hcy and NTproBNP of the stroke group were higher than the control group[(20 ±5) μmol/L vs (12 ±3) μmol/L, (426 ±32) ng/L vs(55 ±9) ng/L],the differences were statistically significant(P<0.05).The serum lev-els of Hcy [(19 ±4) μmol/L,(22 ±6) μmol/L]and NTproBNP [(466 ±69) ng/L,(524 ±73) ng/L] of moderate group and severe group were higher than the mild injury group[(15 ±3) mol/L,(312 ±60) ng/L], the levels of Hcy and NT-proBNP in the severe group were higher than those in the moderate group , the differences were statistically significant ( P <0.05 ) .The levels of serum Hcy [ ( 20 ±4 ) μmol/L, ( 24 ± 5) μmol/L] and NT-proBNP [(487 ±51) ng/L,(544 ±73) ng/L] of the middle infarction group and the large infarction group were higher than the small infarction group[(13 ±3) μmol/L,(285 ±43) ng/L], and the differences were statistically significant(P <0.05).Pearson analysis showed that Hcy,NT-proBNP levels were positively correlated with cerebral infarct volume and NHISS score ( P <0.05 ) .Conclusion Serum levels of Hcy and NT-proBNP are closely related to the neurological deficits and cerebral infarct vol-ume of ischemic stroke,therefore can be used as the reference index for the evaluation of disease condition of ischemic stroke.%目的:研究血清同型半胱氨酸( Hcy)及N端脑钠肽前体( NT-proBNP)与缺血性脑卒中患者神经功能缺损及脑梗死体积的的相关性。方法选取2014年3月至2015年3月榆林市第一医院收治的缺血性脑卒中患者100例作为脑卒中组,同时选取同期健康体检者100例作为对照组。比较脑卒中组及对照组血清Hcy、NT-proBNP水平,分析美国国立卫生研究院卒中量表( NHISS)评分及梗死体积与Hcy、NT-proBNP的相关性。结果脑卒中组患者血清Hcy、NT-proBNP水平显著高于对照组[(20±5)μmol/L比(12±3)μmol/L、(426±32) ng/L 比(55±9) ng/L],差异有统计学意义(P<0.05)。中度及重度损伤组患者血清 Hcy[(19±4)μmol/L、(22±6)μmol/L]、NT-proBNP [(466±69) ng/L、(524±73) ng/L]水平显著高于轻度损伤组[(15±3) mol/L、(312±60) ng/L],且重度组Hcy、NT-proBNP水平高于中度损伤组,差异有统计学意义( P<0.05)。中梗死组和大梗死组患者血清Hcy[(20±4)μmol/L、(24±5)μmol/L]、NT-proBNP[(487±51) ng/L、(544±73) ng/L]水平高于小梗死组[(13±3)μmol/L、(285±43) ng/L],且大梗死组Hcy、NT-proBNP水平高于中梗死组,差异有统计学意义(P<0.05)。经 Pearson分析可知,Hcy、NT-proBNP水平与患者的 NHISS 评分及脑梗死体积呈正相关(P<0.05)。结论血清 Hcy 和 NT-proBNP 水平与缺血性脑卒中患者神经功能缺损程度及脑梗死体积密切相关,因此可作为评价缺血性脑卒中患者病情程度的重要参考指标。
