羟乙基淀粉联合阿托伐他汀治疗分水岭脑梗死的临床疗效

摘要

目的 探讨羟乙基淀粉联合阿托伐他汀治疗分水岭脑梗死(CWI)的临床疗效.方法 选取2016年3月至2017年3月保定市第一中心医院收治的120例CWI患者为研究对象,按照随机数字法分为对照组和试验组,各60例.对照组单纯给予阿托伐他汀口服治疗,试验组在对照组的治疗基础上联用羟乙基淀粉.比较两组患者治疗后的临床疗效,以及治疗前后神经功能缺损评分(NIHSS)、改良Rankin量表评分(mRS)以及日常生活活动(ADL)评分等指标.结果 试验组治疗后总有效率显著高于对照组[83.33%(50/60)比66.67%(40/60),P<0.05].治疗后试验组NIHSS评分显著低于对照组[(3.00±1.09)分比(5.75±1.03)分],ADL评分显著高于对照组[(84.62±8.75)分比(71.36±6.21)分](P<0.01).试验组良好率明显高于对照组[95.00%(57/60)比80.00%(48/60)],并发症发生率显著低于对照组[1.67%(1/60)比10.00%(6/60)](P<0.05).结论 羟乙基淀粉联合阿托伐他汀治疗CWI效果显著,可明显提高CWI的治愈率及好转率,且临床应用安全.%Objective To discuss the effectiveness of hydroxyethyl starch plus atorvastatin in treating cerebral water -shed infarction(CWI).Methods A total of 120 CWI patients admitted to the First Central Hospital of Baoding City from Mar.2016 to Mar.2017 were selected.The patients were assigned to a control group and a experimental group according to the random number method ,60 cases each .All patients were given oral atorvastatin alone ,while the experimental group was also given hydroxyethyl starch .The clinical effect , scores of National Institute of Health Stroke scale ( NIHSS ) , modified Rankin scale(mRS) and activities of daily living (ADL) of the two groups were evaluated .Results After treatment,the effective rate in the experimental group was significantly higher than that of the control group [83.33%(50/60) vs 66.67%(40/60)](P<0.05);after 3 weeks of treatment,the score of NIHSS of the experimental group was lower than that of the control group[(3.00 ±1.09) scores vs (5.75 ±1.03) scores],ADL was higher than that of the control group [(84.62 ± 8.75) scores vs (71.36 ±6.21) scores](P<0.01).The good recovery rate of the experimental group was significantly higher than that of the control group [95.00%(57/60) vs 80.00%(48/60)](P<0.05),the incidence rate of postopera-tive complications in the experimental group was lower than that of the control group [1.67%(1/60) vs 10.00%(6/60)] (P<0.05).Conclusion The hydroxyethyl starch plus atorvastatin can greatly improve the cure rate ,improvement rate and clinical safety in treating CWI .
