腹膜后肿瘤手术前后影像学评估及术前栓塞

         

摘要

Objective To assess the value of many kinds of imageology in pre- and post-operative imaging evaluation and preoperative embolism for retroperitoneal tumor(RPT). Methods Between January 2008 and December 2011, 8 patients with RPT were selected retrospectively. Five of all 8 patients underwent digital subtraction angiography (DSA) for the abdominal aorta and its branch and inferior vena before operate and angei-embolization after identified major feeding artery. One patient underwent phlebography for ilio- femoral vein to understand compensatory circulation after operate. Results Ultrasound diagnosis of 8 cases / 8 cases,CT positioning diagnosis of 8 cases / 8 cases, CT qualitative diagnosis of 6 cases / 8 cases,MRI positioning diagnosis of 5 cases / 5 cases,MRI qualitative diagnosis of 4 cases / S cases, DSA qualitative diagnosis of 4 cases / 5 cases,DSA positioning diagnosis value of poor. Intravenous phelography (IVP) and barium swallow were helpful for clinical diagnosis. Five patients with preoperative embolism were operated successfully with the bleeding of 600-1200ml in tumorectomy. Two patients without preoperative embolism hemorrhaged more than 2000ml in tumorectomy. One patient was only cut the belly open for exploration. Conclusion Unitive imageology is most important and major method for preoperative evaluating RPT. There are significant values for CT and MRI in positioning and qualitation for RPT. DSA is helpful for clinical diagnosis of RPT. Preoperative embolization to reduce bleeding during operation, it has high value in clinical application.%目的 探讨多种影像学手段在腹膜后肿瘤手术前后评估价值和外科术前血管栓塞的临床应用价值.方法 回顾性分析2008年1月~2011年12月间8例腹膜后肿瘤的多种影像学表现,其中5例外科术前行腹主动脉及其分支和下腔静脉数字血管减影,明确主要供血动脉后行血管栓塞术,1例术后作股髂静脉造影了解侧支循环通畅情况.结果 超声诊断8例/8例CT定位诊断8例/8例,定性诊断6例/8例,MRI定位诊断5例/5例,定性诊断4例/5例,DSA定性诊断4例/5例,定位诊断价值较差,IVP、消化道钡餐有助于临床诊断;5例外科术前血管栓塞均获得成功,栓塞者外科术中出血在600~1200ml之间,2例未栓塞者术中出血>2000ml,1例剖腹探查.结论 综合影像学是腹膜后肿瘤术前最重要和主要的评估手段,其中CT、MRI定位、定性价值显著,数字血管减影有助于临床诊断,术前血管检塞达到减少术中出血的目的,具有较高的临床应用价值.
